How to File Online:

Before Claim Filing:
You will need the Unique ID printed on the Notice that you received.

After Claim Filing:
After submitting your completed claim online, you will receive an email with a confirmation code for your completed submission. Be sure to keep your confirmation email and code and refer back to them if you have any questions about your Claim Form.

How to File by Mail or email:

Fill out, sign, and date the Claim Form, then mail the Claim Form to the following address:

Settlement Administrator
P.O. Box 5990
Portland, OR 97228-5990

or email the signed Claim Form to ClaimSubmission@FTDDeliveryFeeSettlement.com.

Remember, all Claim Forms must be submitted online, emailed, or postmarked if mailed to the Settlement Administrator no later than November 13, 2026.
